    Required health forms for students in Beardstown schools should be submitted during registration days, Aug. 5 and 6, from 10 a.m.-7 p.m. or prior to those dates at the school’s office, according to the district administration.
    These include physicals, immunizations, lead screenings, dental exams, vision exams and sports physicals. All students who are required to have health exams were sent a list of what is needed prior to schools’ dismissal in March.  All provider offices have the forms to be filled out.  
    The requirements include:
    • New students to Illinois:  Must have Illinois physical completed and up-to-date immunizations.
    • New to pre-school: Physicals and up-to-date immunizations are required by the first day of school.
    • Kindergarten: Physicals, up-to-date immunizations and lead screens are required by the first day of school. Eye exams are required by Nov. 15, 2020.
    • Kindergarten, second grade, sixth grade and ninth grade: Dental exams are required by May 15, 2021.
    • Sixth grade and ninth grade: Physicals and up-to-date immunizations are required by the first day of school.
    • 12th grade: Up-to-date immunizations are needed by the first day of school.
    Parents should check with their health care provider to make sure students are up to date regarding school health requirements.
    Religious exemptions for immunizations are required the first day of school.
